Attention Q-Club Parents!
 
ONLINE REGISTRATION  FOR Q-CLUB WILL BE ACCEPTED UNTIL FRIDAY, JUNE 15, 2018 at 4:30PM. 
After June 15th, Q-Club registration will be accepted at the department office only.

The department office is open Monday-Friday 8:00am-4:30pm. In order to register, you must bring a current copy of your child’s immunization records, and fill out a registration form and an emergency card (available at the department office). In addition, if your child will be bringing an inhaler, or an epi-pen with them to Q-Club, you must have a Patient Specific Order with you at the time of registration. What Each Child Needs To Register
  • A Completed Registration Form (Available @ Office) or Register Online
  • A Completed Emergency CARD (Available @ Office) or Register Online
  • A Copy of CURRENT immunization records (Obtain yearly from Dr's Office - Required at Registration ~ Fax 518-798-3194)
  • A PATIENT SPECIFIC ORDER (PSO) from your child’s physician, IF your child will be bringing an epi-pen and/or inhaler with them to Q-Club

Additional Information
  • Q-Club is a certified day camp through the NYS Department of Health.
  • To meet state requirements REGISTRATION FORMS WILL NOT BE ACCEPTED WITHOUT A COPY OF:
  • EACH CHILD’S CURRENT IMMUNIZATION RECORDS (INCLUDING A PATIENT SPECIFIC ORDER FOR ALL EPI-PENS AND/OR INHALERS, IF REQUIRED)
  • A COMPLETED REGISTRATION FORM
  • A COMPLETED EMERGENCY CARD
  • Forms currently available for pick-up at the department office or on-line when registering on-line
  • **Q-Club Staff are not responsible for Lost or Stolen Items. Please label all of your child's belongings with both their First and Last Name......ON THE LAST DAY OF CAMP WE DONATE ALL UNCLAIMED ITEMS.
